The GAPhealth mobile application has two portals – one for health practitioners and another for patients with easy sign-up and verification. GAPhealth allows patients to securely communicate with health practitioners on the platform via SMS, phone, and video calls. Patient Portal: The patient interface shown has five primary features: (1) view verified health providers and set up appointments, (2) communicate with providers including receiving post-visit health summary notes, (3) add medical information such as immunizations, medications, lab results, and medical conditions, (4) keep a health journal, (5) view tailored health educational materials.
