Has your health care provider invited you to the Heart for Health app? With this app you can easily engage with your health, from the comfort of your home. Measurements that you submit are automatically forwarded to your care provider so they can monitor your health remotely. Sending measurements to your care provider You can manually submit a measurement or use one of the connected devices to do so. Ask your care provider which devices are available to you. The measurements you submit are automatically forwarded to your care provider. You can also view your current and past measurements within the app. Notifications and reminders You will receive a message within the app when it’s time to perform a measurement.
