Through UPLIN, the MDs can provide virtually all of your healthcare needs in the comfort of your own space (Your Living Room NOT Their Waiting Room!): Sick Visits for children and adults; Primary Care/Family Medicine/Wellness Visits; STD Visits; and Ancillary Services Visits, including blood draws, labwork, IVs, mobile x-rays, mobile ultrasounds, COVID tests (RT-PCR, Antigen & Antibody), etc. UPLIN is also at the forefront of Wearable Technology that will define bespoke medicine! The “Sick” Visit: $600 The “Check-Up/Wellness Visit”: $1200 During Your Visit • Review of personal and family medical history • Review of lifestyle and health choices for Check-Up/Wellness Visit • Evaluate patient concerns • Check of vital signs (blood pressure, heart rate, respiratory rate, pulse oxygen levels, temperature) • A complete, comprehensive physical exam • Blood draw and lab work, e.g., DNA/Genetics, Thyroid, Cholesterol, Diabetes, complete blood count (CBC), age-specific tests, such as PSA, etc. for Check-Up/Wellness Visit • On-site tests, when required, including strep, flu, COVID-19, etc. for Sick Visit • Rendering of diagnosis for Sick Visit • Discuss with patient health conditions and corresponding management • Advise on the best diet and exercise routine for patient Check-Up/Wellness Visit • Address all medical questions • Prescribe medication as required After Your Visit • Appointment summary • Test results • Copy of your treatment plan, if relevant • Documentation you may submit to your health insurance plan or apply toward your HSA/FSA • Chart summary for your primary care physician upon request When Lab Results are Completed • Follow-up communication by the MD to discuss lab results and appropriate management COVID-19 Concerns: Rapid RT-PCR Test: $600 Results In Less Than 24 Hours Urgent RT-PCR Test: $900 Results In Less Than 3 Hours Rapid Antibody Test: $600 Results In Less Than 24 Hours Rapid Antigen Test: $350 Results In 15 Minutes During Your Visit • Check of vital signs (blood pressure, heart rate, respiratory rate, pulse oxygen levels, temperature), as needed • Auscultation (listen to your heart and lungs when needed) • Shallow nasal swab to collect specimen • Blood draw for antibody test After Your Visit  • Test results via email or as directed • Appointment summary • Immediate Treatment plan for patients with COVID-positive results • Prescribe medication as required • Documentation you may submit to your health insurance plan or apply toward your HSA/FSA • Chart Summary for your primary care physician upon request STD Visit in Absolute Confidence (w/ Tests): $850 During Your Visit • Obtain health history • Review of current symptoms and complaints • Check of vital signs (blood pressure, heart rate, respiratory rate, pulse oxygen levels, temperature) • When needed, draw blood for HIV/Herpes/ Syphilis and/or collect urine for Chlamydia/Gonorrhea testing • Review your diagnosis and treatment options • Immediate treatment of your condition • Prescribe medication as required After Your Visit • The MD will discreetly follow up with you regarding lab results and make sure you are doing better • The MD will answer any additional questions you may have concerning your care Infusion Services Whether you’re recovering from an illness or want to boost your immune system, we offer a variety of IV treatments in the comfort of your own place. Blood Draws We offer mobile phlebotomy services in connection with MD visits so that blood draws are done with your convenience in mind.
