100%Cette session a été présentée le 9 avril 2025 Aux prises avec un manque de ressources alors même que le nombre de cas augmentait, le CIUSSS du Centre-Ouest-de-l’Île-de-Montréal a ingénieusement conçu une unité de pratique intégrée (UPI) en faisant participer les patients à la gouvernance et à l’amélioration des processus. Grâce à des efforts de collaboration avec des parties prenantes comme la Coalition Priorité Cancer et le Réseau de cancérologie Rossy, le CIUSSS a établi une approche centrée sur le patient réunissant des médecins cliniciens et des représentants des patients dans tout le continuum des soins. En établissant des indicateurs communs et des processus simplifiés, le CIUSSS a veillé à ce que la responsabilité soit partagée et s’est assuré de surveiller efficacement les résultats. L’importance qu’il a accordée aux résultats et à l’expérience signalés par les patients a permis de réaliser des améliorations tangibles de la prestation des soins. Ce modèle transformateur, en plus d’optimiser l’affectation des ressources, a établi une norme axée sur la valeur pour les soins de santé dans tout le pays.
