Nursing Home Mortality Index app analytics for January 18

The NHMI app was designed to help clinicians better understand their patients' needs and guide treatment planning for older patients. To use this app, simply select the patient's criteria for each category and press "Results". The estimated probability of death in the next 12 months will appear on the screen with a low, moderate, or high risk classification. To clear all criteria selections, press the "Results" button. For more information about the categories, select the information icon to the right of the category title. Tap the link at the bottom of the results page to open the article referenced in this application.
