ImPACT Passport® empowers you to take an active role in concussion management. Provide healthcare professionals access to your data, record symptoms, and locate an ImPACT-trained care provider anytime, anywhere. Grant care providers immediate access to your test results and symptom records by providing them your Passport ID, a unique identification code received upon completion of an ImPACT®, ImPACT Pediatric®, ImPACT Quick Test®, or Cognitive Impairment Screener™ (CIS). ImPACT Passport includes the ability to: • Record ImPACT Passport IDs • Locate ImPACT-trained concussion care providers • Record and upload symptoms to your profile • Grant your healthcare provider access to your test results and symptom records • Generate a Completion Confirmation certificate for any assessment you’ve taken
