To access the app, members will have to complete an initial consultation with their Tria Health pharmacist and register on our patient portal. The member will then have access to their personalized care plan developed by their Tria Health pharmacist at any time. Comprehensive Medication Reminders Medication regimens can vary in complexity, and we know you need more than just a simple alarm to manage it all. Tria Health’s medication reminders allow you to track your medication in any form, at any time of day and ensure you’re aware of when you need a refill. The planner tool allows for greater freedom in customizing your medication schedule. Health Device Dashboards If you are currently using a Tria Health blood glucose meter, blood pressure monitor, or one of our other qualified devices, you can now access a mobile dashboard. You’ll be able to view a summary of your readings, personalize or add notes, customize your health alerts, and order supplies as needed.
