Reya Orthopaedics Remote Care Application is a home monitoring solution for post-op, which involves nurses doctors and even physiotherapists. The application helps the care team manage their patients through their recovery remotely. Eligible patients are onboarded by a nurse on the smartphone app. Patients use the Patient App to add their daily check-ins of vitals and symptoms, if any. Nurses are immediately alerted of adverse information logged in, allowing quick followup with patients. Doctors generate digital care plans for the patient remotely. The system enables care continuity and prompt feedback loops between the patient and their care team members. This App has been integrated with Apple Health App to fetch Walking + Running distance data.
