"My Health Team" app is a iPhone and iPad app into which patients may enter important information regarding their doctors and other health care providers including their names, pictures, specialities, emergency phone numbers, non-emergency phone numbers, and email addresses. Multiple health care providers can be added manually, or if the app is being used by a health system, the health system may purchase from Salient Care, LLC software to allow the provider information to be airdropped into patients' iPhone or iPad directly. To call or email their doctor or other provider, patients just need to touch the provider's face and a list of phone numbers and email links drop down. When they touch the telephone number, the iPhone dials the number. When they touch the email address (if it is correct) a blank email appears with the outgoing address and they can them type and send their message to their provider. The idea is to move away from paper business cards to the ease of a smartphone, allowing patients, families, and other providers to understand the team composition and reach out to any of those Health Team Members at a moments notice (e.g., in an outside emergency room, at home when they are feeling ill, if they want to schedule an appointment). It was developed with input from patients with cancer who have many doctors and other types of providers caring for them at all time. They said that they had so many business cards but did not know who their team of doctors were or how to reach them in emergencies and in non-emergent situations. That is why we created the My Health Team app. We think it will help patients, family members, and patients' providers to communicate in a more informed and timely fashion than previously which we hope will lead to greater patient satisfaction, healthier patients overall, and fewer visits to emergency rooms and the hospitals.
