Reya CDM Application App-Analysen für 13. Januar

Reya Chronic Disease Management App helps nurses and doctors manage their patients’ chronic diseases remotely. Eligible patients are on-boarded by a nurse on the smartphone app. Patients use the Patient App to add vitals and symptoms. Nurses are immediately alerted of severity of symptoms, allowing quick followup with patients. Doctors generate digital care plan for the patient remotely. The system enables care continuity and prompt feedback loops between the patient and their care team. The app also helps in Task Segregation of clinical & non-clinical requests and redirection to appropriate Care Team members. Overall, this leads to better patient experience, and improved quality of care.  This App has been integrated with Apple Health App to fetch Walking + Running distance data.
