Fold Care App helps you stay engaged in your health journey by making it easier to manage your care, communicate with your providers, join virtual visits, and track your progress over time. Practices using Fold offer an optional wellness feature that allows you to connect your smartphone or wearable device through Health Connect. If enabled for your care program, you may choose to share activity and wellness information - such as steps, active calories, exercise sessions, heart-rate data, sleep patterns, blood glucose, or oxygen saturation. Sharing this information is completely optional and not required to use the app. When you choose to connect a device, these insights can help you and your care team better understand your overall wellness trends and support your care plan. Fold only requests the minimum Health Connect data types needed for the features you choose to use. All data is read-only, handled securely, used only to support your care experience, and never used for advertising. You can change or revoke access at any time in your device settings.
