The SDHC officially launched on 17th October 2022, located in Wong Chuk Hang, with satellite centers in Stanley, Lei Tung, Chi Fu, and Aberdeen, as well as a Health Library in Ap Lei Chau. SDHC utilizes information technology to connect healthcare professionals in the district and implements various primary health services such as the Life Course Preventive Care (LCPC), Chronic Disease Co-Care Pilot Scheme (CDCC), and the Family Doctor Pairing (FD). It also strengthens coordination with other primary healthcare services and facilities in different regions to provide a convenient environment for residents to care for their personal health. The mobile app, "Health Navigator", offers the following features to facilitate information access and enhance user experience. More features will be added in the future.
