Down Syndrome Health Record delivers the following features: - Record information relating to medical diagnoses, allergies, medications, vaccinations, hospital admissions and healthcare providers - Monitor weight, length/height and head circumference, as well as developmental milestones - Track active symptoms and share information with healthcare providers - Record and prepare for healthcare appointments Other features include: - Generate a Health Record Summary that can be exported as a PDF and shared with others (e.g. healthcare professionals) - Upload or scan documents (e.g. discharge summaries, referral letters, prescriptions, etc) - Export and share data with another person (e.g. family member, support person) who can then import the data to open on their own device
