The app helps the Provider Organizations deliver relevant health information to the community users based on age, health issues, disease outbreaks, etc. With this app, you are always connected with your Provider Organization and receive the following benefits: - Get latest community news related to health issues (outbreaks etc.) - Receive research, evidence, practice-based insights, and other forms of health information - Collect real-time data to help identify and respond to acute outbreaks, emergencies, and other health hazards. - Connect health and social services that support the whole person, including preventive services. - Ensuring access to high-quality and cost-effective healthcare and social services, including behavioral and mental health services, that are culturally and linguistically appropriate - Provide information to your provider to monitor your health status in real time and to identify patterns to develop strategies to address chronic diseases and injuries - Securely communicate with your provider and other support resources in real time through an interactive chat WHAT DO WE DO? Aztute is a public health platform that helps DETECT potential health and environmental issues, effectively RESPOND with the appropriate action, deploy successful policy and interventions to RECOVER from a crisis, and institute processes to PREVENT potential crises from occurring in the future. It does this by enabling public health to collaborate with community partners, residents, and other stakeholders to create a powerful real time data platform that enables quick collection and analysis of community data to identify threats, patterns, and emerging issues. HOW DO I GET THE APP? To access Aztute, you must create an account with your Provider Organization. To sign up for an account, download the app and search for your healthcare organization (by typing Tenant Name or website) in the app or go to your healthcare organization’s website.
