Will I be able to keep the same Medical Home when I reapply? In most cases, yes. Make sure to ask your HSF Application Assistor during your enrollment appointment. Why do I have to renew? Each year you must reapply for HSF. Without reapplying, you will lose your health care coverage and access to services. To reapply, you must meet with an HSF Application Assistor to fill out a new application every year. You must reapply with updated documents.
