Comunicare aims to support patients in the context of chronic diseases by providing organizational support. It provides personalized information on the patient care plan, appointments and recommendations for a better quality of life. All therapeutic information is provided by the medical team. The patient can share most of this information with his entourage. In return, the patient is asked to regularly share his outcomes and vital signs with the medical team for a better evaluation of the therapy. This app integrates Apple HealthKit (on iPhone only) to help monitor physical activity. A video module allows to organize teleconsultations with the medical team. The application allows the integration of measurements of vital parameters taken via Bluetooth from validated medical devices (tensiometer, thermometer, oximeter, scale).
