The To-Do list is based on the data in your Care Plan and combines all of your information, such as doctor appointments and medication refills, in one platform to make it convenient and easy to remember the important things for your health. Whether it is to remind you to take your medication or exercise or monitor your vitals, an automatic notification will be sent to you ten minutes prior to the hour that the task is due REACH YOUR HEALTH GOALS It is important to know every part of your body and how it reacts to different exercise and medication in order to be able to make the best possible decisions in regards to your health. How much sleep do you need? Do you know what you are eating and how it affects your weight? If you are more active will it have a positive impact on your blood sugar level? With MyCarePlan app you will be able to closely monitor your vital health data on a user-friendly set of graphs, and better understand the meaning behind your numbers. The Care Plan includes a lifestyle Health test, where the results allow for you to see how healthy you have been living during the past week. The test is quick and the results come with a recommendation on how to improve the aspects of your health which are not optimized as well as a praise on those which the requirements are met. The results of your health test are also important for your providers, because it allows them to see which aspects of your lifestyle can be improved by improving your lifestyle habits.
