Using this convenient service, you can talk to an experienced Memorial Care provider via secure video link and get a diagnosis and treatment plan without leaving your home. Patient Portal Use FollowMyHealth or DMH Chart to access your health records, view test and lab results, request prescription renewals, talk to your doctor and more. How It Works The first step is to create a profile within the Memorial Care app. This profile will include your name, birthdate and basic contact info. Your profile also allows you to add dependents and create emergency contacts. Your dependents and emergency contacts can also save their spot in line using your Memorial profile.
