If your school or sports organization has assigned you a test or given you a test code, download ImPACT Passport to securely complete your assessment. ImPACT Passport empowers you to take an active role in concussion management. Provide healthcare professionals access to your data, record symptoms, and locate an ImPACT-trained care provider anytime, anywhere. Grant care providers immediate access to your test results and symptom records by providing them your Passport ID, a unique identification code received upon completion of an ImPACT®, ImPACT Pediatric®, ImPACT Quick Test®, or Cognitive Impairment Screener™ (CIS). ImPACT Passport includes the ability to: • Record ImPACT Passport IDs • Locate ImPACT-trained concussion care providers • Record and upload symptoms to your profile • Grant your healthcare provider access to your test results and symptom records • Generate a Completion Confirmation certificate for any assessment you’ve taken
